Adopt an Elder Foundation (AAEF) asks you to provide personal information such as name, email address, phone number and zip code, and medical information, and may or may not include any other information provided by or for you, and/or provided by any other person for whom you may be seeking senior housing and/or care services. We use Personal Information to help us create an appropriate senior care services that can accommodate all your individual needs. This information is kept strictly confidential by both Life Options for Seniors and the senior care service providers referred to.

We may disclose or share Personal Information about you with other senior service providers including:
Contracting Communities. We will only disclose or share Personal Information to one or more Contracting Communities with your permission in order to provide you with the services you’ve requested. The Contracting Communities may contact you directly by email, telephone, regular mail. We do not control how these Contracting Communities use or disclose your Personal Information.
In-Home Care Providers. We will only disclose or share Personal Information to one or more In-Home Care Providers with your permission in order to provide you with the services you’ve requested. The In-Home Care Providers may contact you directly by email, telephone, regular mail. We do not control how these Contracting Communities use or disclose your Personal Information.
Business Transfers. We may disclose Personal Information as part of any merger, acquisition and sale of company assets or other business transaction.
